Dr. Motameni is a USCIS-designated civil surgeon. We walk you through what to bring, which vaccines and tests are needed, and how your sealed form is returned — in English, Spanish or Farsi.

What to bring

  • Passport or other government photo ID
  • Your vaccination records
  • A list of your current medications
  • Any USCIS letters or notices about your case
  • Your insurance card, if you have one
